Notice of Privacy Practices
You deserve to know how your child's health information is protected — and what rights you have.
At Growing Gains Therapy, the same honesty and clarity we bring to your child's care extends to how we handle your family's private information. This notice explains how we may use and share your child's health information, and the rights you have over it.

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU (OR YOUR CHILD) MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Our Commitment to Your Privacy
We are committed to protecting the privacy of the health information we create and maintain about your child. This information — sometimes called Protected Health Information, or "PHI" — may include evaluation results, treatment notes, progress summaries, and similar records of your child's care.
We are required by law to:
- Keep your child's protected health information private;
- Give you this notice of our legal duties and privacy practices regarding that information; and
- Follow the terms of the notice currently in effect.
How We May Use and Share Health Information Without Your Authorization
For Treatment. We use your child's health information to provide and coordinate care. For example, with your involvement, we may share information with your child's physician, teachers, IEP team, or other specialists to support one unified plan — a core part of how we work.
For Payment. Because we are a private-pay practice and do not bill insurance, we generally use your information for payment only to process your payments and to prepare detailed superbills that we provide to you. If you choose to submit a superbill to your insurance plan for out-of-network reimbursement, you are sharing that information with your insurer — we do not submit claims to insurers on your behalf.
For Health Care Operations. We may use health information to run our practice well — for example, to review and improve the quality of our services, train and supervise clinicians, and conduct internal administrative activities.
Appointment Reminders and Care Communications. We may contact you with appointment reminders, weekly written session summaries, and information about treatment options or services that may benefit your child.
To Those Involved in Your Child's Care. With your permission, or when you don't object, we may share relevant information with a family member, caregiver, or other person you've identified as involved in your child's care.
Uses and Disclosures We Are Permitted or Required to Make by Law
Subject to limits in the law, we may use or share health information without your authorization when:
- Required by law, including federal, state, or local reporting obligations;
- Reporting suspected child abuse or neglect — as mandated reporters working with children, we are legally required to report suspected abuse or neglect to the appropriate authorities;
- Needed for public health activities, such as preventing or controlling disease;
- Requested for health oversight activities, such as audits, investigations, or licensure;
- Required in judicial or administrative proceedings, such as in response to a court order or, with appropriate safeguards, a subpoena;
- Requested for law enforcement purposes as permitted by law;
- Necessary to prevent a serious and imminent threat to the health or safety of your child or others;
- Required for workers' compensation, specialized government functions, or to coroners or medical examiners as authorized by law.
Uses and Disclosures That Require Your Written Authorization
Most uses and disclosures not described above will be made only with your written authorization. In particular:
- We will not use or share your child's health information for marketing purposes without your written authorization.
- We will not sell your child's health information.
You may revoke an authorization in writing at any time, except to the extent we have already acted in reliance on it.
A Note on Children and Parental Access
Because we serve children, a parent or legal guardian is generally the child's "personal representative" and may exercise the rights described in this notice on the child's behalf, including accessing the child's records. In limited circumstances where the law allows a minor to consent to their own care, or where doing so could endanger the child, access may be limited as required by law. We're glad to discuss how this applies to your family.
Your Rights Regarding Your Child's Health Information
You have the right to:
- Request restrictions on how we use or share your child's health information for treatment, payment, or operations. We are not required to agree to every request, but we'll consider each one.
- Request confidential communications — for example, asking us to contact you at a specific phone number or email, or by a particular method. We'll accommodate reasonable requests.
- Inspect and receive a copy of your child's records, in paper or electronic form, usually within 30 days of a written request. We may charge a reasonable, cost-based fee.
- Request a correction or amendment if you believe information in the record is incorrect or incomplete. We may deny the request in certain cases, and will explain why in writing.
- Receive an accounting of certain disclosures we've made of your child's health information.
- Receive a paper or electronic copy of this notice at any time, even if you've agreed to receive it electronically.
- Be notified if there is a breach of your child's unsecured health information.
To exercise any of these rights, please contact us using the information below. Most requests should be made in writing.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us using the contact information below, or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be penalized or retaliated against for filing a complaint.
Changes to This Notice
We reserve the right to change this notice and to make the revised notice effective for all health information we maintain. The current notice will always be available upon request and posted on our website, with an updated effective date.
